POSITION:
As an integral member of the Social Work team, the Transplant Resource Specialist helps patients/families/support persons access resources for listing and for pre/post-transplant. The responsibilities include: providing assistance to patients/families/support persons and hospital staff on a consultation basis regarding transplant related resources and other community resources (e.g. Trillium Drug Program, Accommodation, Transportation, etc.); assisting with application forms and letter writing (e.g. TDP, Expedite, MORRR, ODSP, EI, CPP-D, Wheeltrans, Immigration Visa, Housing, etc.); staff education and updating resources used by staff and patients/families/support persons; acting as a liaison between the hospital and community agencies; assisting staff with program planning projects (e.g. developing education materials and group teaching).
QUALIFICATIONS:
- Social Service Worker Diploma
- Registration with the Ontario College of Social Workers and Social Service Workers
- 3 years relevant work experience
- Knowledge of Community and Government Resources (e.g. income replacement, drug coverage, transportation)
- Community health experience
- Teaching experience
- Familiarity with principles of Adult Education
- Presentation skills
- Strong Communication skills (written and verbal)
- Computer skills (e.g. Word, Excel, Outlook)
