As an integral member of the Antimicrobial Stewardship Team, the Antimicrobial Stewardship Pharmacotherapy Specialist plays a key role in establishing initiatives that foster improved antimicrobial use, lower antimicrobial resistance, increased patient safety and cost-savings across all sites of the University Health Network and Sinai Health. The Antimicrobial Stewardship Pharmacotherapy Specialist will work with the ASP Team to establish guidelines that ensure appropriate antimicrobial therapy including: antibiotic prophylaxis, establishing guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy associated with a definite diagnosis, optimizing antimicrobial dose and route of administration as well as duration of antimicrobial therapy. You will work in collaboration with the Antimicrobial Stewardship Infectious Disease physicians and our inter-professional stakeholders to provide implementation strategies for initiatives including: education to hospital staff, prospective and retrospective evaluation of ongoing compliance and provide feedback. You will assist in ongoing antimicrobial and drug surveillance reporting, and you will participate in antimicrobial utilization quality initiatives in conjunction with the hospital’s continuous quality improvement initiatives. With your experience and knowledge of research methodology you identify opportunities for antimicrobial stewardship and inter-professional research activities. As a leader, you will be responsible for providing education and feedback to pharmacists and physicians regarding appropriate use of antimicrobial agents and represent the ASP at the local, provincial and national level.The Pharmacotherapy Specialist is responsible for supervising University of Toronto PharmD students during clinical rotations as well as undergraduate and residency programs; perform cross-functional responsibilities and cross-coverage with Sinai Health as it relates to ASP activities; perform other duties and assume responsibilities consistent with the job classification. The successful candidate should have excellent interpersonal and leadership skills and be adept at adapting to various work environments. The position requires an individual who is self-motivated, creative, and can work independently and as a member of an inter-professional team. Individuals applying for this position should have an advanced degree with infectious disease training or equivalent clinical experience.

QUALIFICATIONS

  • Completion of a Bachelor of Science degree in Pharmacy from an accredited program of study required
  • Completion of post-graduate Pharmacy degree (e.g. clinical Master’s degree or Doctor of Pharmacy) required
  • Advanced clinical degree with infectious disease training or equivalent clinical experience required
  • Active registration with the Ontario College of Pharmacists required
  • Minimum of three (3) years of hospital experience required
  • Previous antimicrobial stewardship experience preferred
  • Excellent verbal and written communication skills
  • Excellent interpersonal, organizational and time management skills
  • Experience in applying change management frameworks
